外国人工作许可证补办业务申办指引

　　《外国人工作许可证》遗失或损毁的，申请人应当自证件遗失（损毁）之日或发现遗失（损毁）之日起，先在“系统”上提出补办申请。

　　一、办理程序 

　　网上申请→ 网上预审→ 受理→ 审查→ 决定

　　外国人来华工作管理服务系统官网地址：https://fwp.safea.gov.cn/

　　二、材料清单及要求

　　所需材料如下：

　　1.《外国人来华工作许可补办申请表》（系统中填写好申请人信息,在线导出申请表,打印后需申请人签名，加盖单位公章）；

　　2.申请人遗失或毁损情况说明；

　　3.遗失公告截图（需先在外国人来华工作管理服务系统上申请发布遗失公告）。

　Guide for Re-issuance of Foreigner’s Work Permit of FWP

　　In case of loss/damage of a FWP, the applicant shall make an application for re-issuance since the date of loss/damage.

　　2. Checklist and Requirements

　　The documents required are listed as follows:

　　2.1 Application Form for Replacement of Foreigner’s Work Permit (the applicant should fill in his/her personal information in the system, download the completed application form, print it, sign it and stamp it with the official seal of the employer)

　　2.2 A description of the loss or destruction of the applicant’s work permit.

　　2.3 Screenshot of the notice of loss (the notice of loss must be posted on the Service System for Foreigners Working in China first).
